Summary

Fluoroquinolones and systemic corticosteroids each raise the risk of tendinopathy, and using them together increases the chance of tendon inflammation or rupture, especially the Achilles tendon. Risk is higher in older patients and those on prolonged steroid courses. With an ophthalmic or otic dexamethasone-tobramycin product, systemic exposure is low and the risk is correspondingly smaller.

What this interaction means in plain words

Dexamethasone-tobramycin and Moxifloxacin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Dexamethasone-tobramycin belongs to Anti Bacterial; Moxifloxacin belongs to Anti Bacterial.

Evidence level: Moderate

Why it happens

Additive pharmacodynamic tendon toxicity from fluoroquinolone-related collagen and tenocyte injury combined with corticosteroid-induced tendon weakening.

What to do

Counsel the patient to stop moxifloxacin and rest the limb if tendon pain or swelling develops. Prefer a non-fluoroquinolone antibiotic in older patients or those on prolonged systemic steroids.
