Summary

Dexamethasone is a weak-to-moderate CYP3A4 inducer and may modestly lower rivaroxaban concentrations, potentially reducing anticoagulant protection. The effect is generally smaller than that of combined strong P-gp and CYP3A4 inducers. Systemic corticosteroids can also add to bleeding risk through gastric mucosal injury.

What this interaction means in plain words

Dexamethasone-tobramycin and Rivaroxaban can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Dexamethasone-tobramycin belongs to Anti Bacterial; Rivaroxaban belongs to Cardiovascular.

Evidence level: Low

Why it happens

Dexamethasone induces CYP3A4 and possibly P-gp, increasing rivaroxaban clearance.

What to do

Monitor for signs of thrombosis or reduced anticoagulant effect during prolonged dexamethasone courses, and watch for GI bleeding. Reassess the anticoagulant choice if therapy is long term.
