Summary

Rifapentine induces hepatic enzymes and can substantially lower diazepam concentrations, reducing anxiolytic, sedative or anticonvulsant effect. Effects may persist for some time after rifapentine is stopped.

What this interaction means in plain words

Diazepam and Rifapentine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Diazepam belongs to Musculoskeletal; Rifapentine belongs to Anti Bacterial.

Evidence level: Moderate

Why it happens

Induction of CYP3A4 and CYP2C19 by rifapentine, increasing diazepam clearance.

What to do

Monitor clinical response and increase the diazepam dose if effect is lost. Reduce the dose again when rifapentine is discontinued to avoid accumulation.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
