Summary
Rifapentine is a strong CYP3A4 inducer and is expected to markedly lower ranolazine concentrations, causing loss of antianginal efficacy. Ranolazine labeling contraindicates CYP3A inducers.
What this interaction means in plain words
Ranolazine and Rifapentine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Ranolazine belongs to Cardiovascular; Rifapentine belongs to Anti Bacterial.
Why it happens
Strong induction of CYP3A4 (and P-gp) increases ranolazine clearance.
What to do
Do not co-administer. Use an alternative antianginal during and shortly after rifapentine therapy.
What to ask your pharmacist
- Is there an alternative to one of these two medicines for me?
- If both are essential, who is monitoring me and how often?
- Which symptoms mean I should stop and seek help straight away?
