Summary
Diclofenac can reduce the antihypertensive effect of valsartan and hydrochlorothiazide. In older or volume-depleted patients, the combination of an NSAID, diuretic and ARB can cause acute kidney injury and hyperkalemia.
What this interaction means in plain words
Diclofenac and Hydrochlorothiazide-valsartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Diclofenac belongs to Musculoskeletal; Hydrochlorothiazide-valsartan belongs to Cardiovascular.
Why it happens
NSAID inhibition of renal prostaglandin synthesis reduces renal blood flow and sodium excretion, and combined with ARB and diuretic effects impairs glomerular filtration.
What to do
Monitor blood pressure, creatinine and potassium after starting diclofenac, and maintain hydration. Use the lowest effective dose for the shortest time or choose an alternative analgesic.
What to ask your pharmacist
