Overview
Concurrent systemic NSAID use increases risk of GI bleeding/ulceration, renal injury, and CV thrombotic events with minimal added analgesic benefit.
Mechanism of action
Additive COX-1/COX-2 inhibition reduces protective prostaglandins → GI mucosal injury and decreased renal perfusion; overlapping NSAID toxicities.
Clinical management
Avoid concurrent systemic NSAIDs; use a single NSAID at the lowest effective dose for the shortest duration. If unavoidable, add PPI gastroprotection and monitor for bleeding, renal function (SCr, BUN), BP, and edema.
