Summary

Oral gentamicin could in theory alter gut flora and reduce enterohepatic recycling of estrogens. Systemic gentamicin does not meaningfully affect gut flora, and clinical evidence of reduced hormone effect is lacking. The risk of contraceptive failure is considered very low.

What this interaction means in plain words

Dienogest / estradiol valerate and Gentamicin have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Evidence level: Low

Why it happens

Theoretical reduction in gut bacterial deconjugation of estrogens and so less enterohepatic recirculation; not established for aminoglycosides.

What to do

No routine action is needed. Counsel the patient to report breakthrough bleeding, and consider backup contraception only if prolonged oral antibiotic therapy causes vomiting or diarrhea.
