Summary

Diflunisal can attenuate the blood-pressure-lowering effect of eprosartan and hydrochlorothiazide. With a diuretic and an angiotensin receptor blocker, NSAID use also increases the risk of acute kidney injury and hyperkalemia, especially in older or volume-depleted patients.

What this interaction means in plain words

Diflunisal and Eprosartan-hydrochlorothiazide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Diflunisal belongs to Musculoskeletal; Eprosartan-hydrochlorothiazide belongs to Cardiovascular.

Evidence level: High

Why it happens

Inhibition of renal prostaglandin synthesis causes sodium and water retention and reduced renal perfusion, opposing antihypertensive effects and impairing glomerular autoregulation.

What to do

Use the lowest effective NSAID dose for the shortest duration and keep the patient hydrated. Monitor blood pressure, serum creatinine and potassium.
