Summary
NSAIDs can blunt the antihypertensive effect of angiotensin receptor blockers and, together, can cause acute kidney injury and hyperkalemia. The risk is greatest in older, volume-depleted or renally impaired patients.
What this interaction means in plain words
Diflunisal and Eprosartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Diflunisal belongs to Musculoskeletal; Eprosartan belongs to Cardiovascular.
Why it happens
NSAID inhibition of vasodilatory renal prostaglandins reduces glomerular perfusion while ARB blocks efferent arteriolar tone, impairing filtration and potassium excretion.
What to do
Monitor blood pressure, serum creatinine and potassium after starting or changing the NSAID, and keep the patient hydrated. Use the shortest NSAID course possible.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
