Summary
Diflunisal can blunt the blood pressure lowering effect of trandolapril. Together they may also reduce glomerular filtration and cause acute kidney injury, especially in older, volume-depleted or renally impaired patients.
What this interaction means in plain words
Diflunisal and Trandolapril can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Diflunisal belongs to Musculoskeletal; Trandolapril belongs to Cardiovascular.
Why it happens
NSAID inhibition of renal vasodilatory prostaglandins opposes ACE inhibitor effects and reduces glomerular perfusion.
What to do
Monitor blood pressure, serum creatinine and potassium when starting or changing the NSAID, and keep the patient well hydrated. Use the lowest effective NSAID dose for the shortest time or choose paracetamol.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
