Summary
Diltiazem markedly increases nisoldipine exposure, which has low oral bioavailability due to extensive first-pass CYP3A4 metabolism. Excessive vasodilation can cause hypotension, flushing, headache and peripheral edema, in addition to additive antihypertensive effects.
What this interaction means in plain words
Diltiazem and Nisoldipine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Diltiazem belongs to Cardiovascular; Nisoldipine belongs to Cardiovascular.
Why it happens
Diltiazem inhibits intestinal and hepatic CYP3A4, and both drugs are calcium channel blockers with additive vasodilatory effects.
What to do
Start nisoldipine at a low dose and titrate cautiously according to blood pressure, or choose another agent. Monitor for hypotension and edema.
