Summary

Both drugs are CYP3A4 substrates and inhibitors, so combining them can raise concentrations of either one. Higher diltiazem exposure may cause bradycardia, AV block or hypotension, and tipranavir exposure may also rise. Tipranavir is always given with ritonavir, which adds to the inhibition.

What this interaction means in plain words

Diltiazem and Tipranavir can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Diltiazem belongs to Cardiovascular; Tipranavir belongs to Anti Virals.

Evidence level: Moderate

Why it happens

Mutual inhibition of CYP3A4-mediated metabolism, with additional inhibition from ritonavir.

What to do

Monitor heart rate, ECG PR interval and blood pressure, and reduce the diltiazem dose if bradycardia or hypotension occurs. Also monitor for tipranavir adverse effects such as hepatotoxicity.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
