Summary
Tipranavir, which is given with ritonavir, strongly inhibits CYP3A4 and can markedly raise avanafil exposure. This may cause profound hypotension, syncope, visual disturbances and prolonged erection or priapism.
What this interaction means in plain words
Avanafil and Tipranavir are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Avanafil belongs to Genitourinary; Tipranavir belongs to Anti Virals.
Why it happens
Ritonavir-boosted tipranavir inhibits CYP3A4-mediated first-pass and hepatic metabolism of avanafil, reducing its clearance.
What to do
Do not combine avanafil with ritonavir-boosted tipranavir. Choose a non-CYP3A4-dependent option for erectile dysfunction, or use a PDE5 inhibitor at a labeled reduced dose with specialist advice.
