Summary
Tricyclic antidepressants such as doxepin can alter the cardiovascular response to sympathomimetic amines. Dobutamine acts mainly directly on beta-1 receptors, so the effect is less pronounced than with indirect agents, but exaggerated pressor responses, tachycardia and arrhythmias are possible.
What this interaction means in plain words
Dobutamine and Doxepin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Dobutamine belongs to Cardiovascular; Doxepin belongs to Dermatology.
Why it happens
Doxepin inhibits neuronal norepinephrine reuptake, potentiating adrenergic stimulation, and its alpha-blocking and anticholinergic effects may further alter hemodynamics.
What to do
Monitor blood pressure, heart rate and ECG during dobutamine infusion and titrate to response using the lowest effective dose. Be ready to reduce the infusion rate if hypertension or arrhythmias occur.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
