Summary

Doxepin inhibits serotonin and norepinephrine reuptake, and combining a tricyclic with an MAOI can cause serotonin syndrome or hypertensive crisis. Some specialist regimens use cautious combinations, but routine use is hazardous.

What this interaction means in plain words

Doxepin and Tranylcypromine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Doxepin belongs to Dermatology; Tranylcypromine belongs to Central Nervous System.

Evidence level: High

Why it happens

Reuptake inhibition plus irreversible MAO inhibition increases synaptic serotonin and norepinephrine.

What to do

Avoid the combination. Allow a washout of at least 14 days after stopping tranylcypromine before starting doxepin, and a suitable washout in the other direction.
