Summary
Dobutamine is partly inactivated by catechol-O-methyltransferase (COMT). Tolcapone inhibits COMT, which may prolong and enhance dobutamine's effects and lead to tachycardia, arrhythmia or blood pressure changes.
What this interaction means in plain words
Dobutamine and Tolcapone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Dobutamine belongs to Cardiovascular; Tolcapone belongs to Central Nervous System.
Why it happens
Tolcapone inhibits COMT, reducing metabolic clearance of the catecholamine dobutamine.
What to do
Monitor heart rate, rhythm and blood pressure closely during dobutamine infusion in patients on tolcapone. Use lower starting infusion rates and titrate cautiously.
