Summary
Tolcapone inhibits COMT, and with a nonselective MAO inhibitor both main catecholamine degradation routes are blocked. This can cause dangerous increases in heart rate and blood pressure, and labeling contraindicates the combination.
What this interaction means in plain words
Tolcapone and Tranylcypromine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Tolcapone belongs to Central Nervous System; Tranylcypromine belongs to Central Nervous System.
Why it happens
Combined COMT and MAO inhibition prevents catecholamine metabolism.
What to do
Do not co-administer. Use an alternative adjunct for parkinsonian therapy if tranylcypromine is needed.
