Summary
Donepezil is metabolized in part by CYP3A4 and CYP2D6. Ribociclib may raise donepezil levels, increasing cholinergic effects such as nausea, bradycardia and syncope. Donepezil can also prolong QT, adding to ribociclib's QT effect.
What this interaction means in plain words
Donepezil transdermal and Ribociclib can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
CYP3A4 inhibition by ribociclib increases donepezil exposure, with additive QT effects.
What to do
Monitor heart rate, ECG and electrolytes, and watch for cholinergic adverse effects. Reduce donepezil dose if these occur.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
