Summary

Ribociclib is a moderate CYP3A4 inhibitor and also prolongs the QT interval in a dose-dependent manner. Combined with cisapride, which is a CYP3A4 substrate with a known risk of torsades de pointes, this could produce serious arrhythmia.

What this interaction means in plain words

Cisapride and Ribociclib are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Cisapride belongs to Gastrointestinal Tract; Ribociclib belongs to Oncology.

Evidence level: Moderate

Why it happens

Ribociclib inhibits CYP3A4, which may increase cisapride exposure, and both drugs prolong the QT interval additively.

What to do

Avoid the combination and choose a non-QT-prolonging prokinetic or antiemetic. If it cannot be avoided, specialist supervision with ECG and electrolyte monitoring is required.

What to ask your pharmacist

  • Is there an alternative to one of these two medicines for me?
  • If both are essential, who is monitoring me and how often?
  • Which symptoms mean I should stop and seek help straight away?
