Summary

Elranatamab causes cytokine release syndrome, and raised cytokines can temporarily suppress CYP enzyme activity. This may increase fentanyl exposure and opioid effects, mainly during step-up dosing and around CRS episodes.

What this interaction means in plain words

Elranatamab and Fentanyl can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Elranatamab belongs to Oncology; Fentanyl belongs to Musculoskeletal.

Evidence level: Low

Why it happens

Cytokine-mediated suppression of CYP3A4 reduces fentanyl metabolism.

What to do

Monitor for excess sedation and respiratory depression during step-up dosing and for about 2 weeks after the first treatment dose, and during CRS. Lower the fentanyl dose if needed.
