Summary
Elranatamab can trigger cytokine release syndrome, and the resulting inflammation suppresses hepatic CYP enzymes. Clozapine is mainly cleared by CYP1A2, so its levels may rise and cause sedation, seizures, hypotension or cardiac effects. The risk is greatest during step-up dosing and around any CRS episode.
What this interaction means in plain words
Clozapine and Elranatamab can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Clozapine belongs to Central Nervous System; Elranatamab belongs to Oncology.
Why it happens
Cytokine-mediated suppression of CYP1A2 reduces clozapine clearance.
What to do
Monitor for clozapine toxicity and consider checking clozapine levels during step-up dosing and for about two weeks after, especially during CRS. Reduce the clozapine dose if signs of toxicity appear.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
