Summary

Elranatamab induces cytokine release syndrome, and elevated cytokines can temporarily suppress CYP enzyme activity. This may raise olanzapine exposure, which is cleared partly by CYP1A2, and increase sedation or other adverse effects.

What this interaction means in plain words

Elranatamab and Olanzapine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Elranatamab belongs to Oncology; Olanzapine belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Cytokine-mediated suppression of CYP1A2 metabolism of olanzapine.

What to do

Monitor for olanzapine adverse effects such as sedation and hypotension during step-up dosing and for about 14 days after, and during any CRS episode. Reduce the olanzapine dose if toxicity appears.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
