Summary
Cytokine release syndrome from elranatamab can transiently suppress CYP enzymes, including CYP1A2, which can increase theophylline exposure. This risk is greatest during step-up dosing and for some days after. A narrow therapeutic index makes toxicity a concern.
What this interaction means in plain words
Elranatamab and Theophylline can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Elranatamab belongs to Oncology; Theophylline belongs to Respiratory.
Why it happens
Cytokine-mediated suppression of CYP1A2 and CYP3A4 reduces theophylline clearance.
What to do
Monitor theophylline concentrations and for toxicity during step-up dosing and any CRS, and reduce the dose as needed.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
