Summary

Tenofovir disoproxil is renally eliminated and can cause proximal tubular injury, and cobicistat raises serum creatinine by inhibiting tubular secretion. Gentamicin is itself nephrotoxic and ototoxic, so combined use raises the risk of acute kidney injury and aminoglycoside accumulation.

What this interaction means in plain words

Elvitegravir / cobicistat / emtricitabine / tenofovir df and Gentamicin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Evidence level: Moderate

Why it happens

Additive nephrotoxicity from tenofovir tubular toxicity and aminoglycoside accumulation in proximal tubular cells, with reduced aminoglycoside clearance if renal function falls.

What to do

Avoid concurrent use where possible, or monitor serum creatinine, phosphate and urine protein frequently and measure gentamicin levels. Stop or change therapy if renal function worsens.
