Summary
Tenofovir is eliminated by the kidney, and NSAIDs can reduce renal perfusion, which may increase tenofovir exposure and the risk of kidney injury. Cobicistat also raises serum creatinine by inhibiting tubular secretion, which can complicate assessment of renal function.
What this interaction means in plain words
Elvitegravir / cobicistat / emtricitabine / tenofovir df and Mefenamic acid can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
NSAID-induced reduction in renal prostaglandin-dependent perfusion and competition for renal elimination, adding to tenofovir tubular toxicity risk.
What to do
Avoid chronic or high-dose NSAID use and choose an alternative analgesic such as paracetamol; if unavoidable, monitor creatinine clearance and urinary markers regularly.
