Summary

Corticosteroids can cause gastric mucosal injury and capillary fragility, which may add to bleeding risk in patients on low molecular weight heparin. Effects on coagulation are variable and poorly quantified. INR is not the appropriate monitoring test for enoxaparin.

What this interaction means in plain words

Enoxaparin and Prednisone have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Enoxaparin belongs to Cardiovascular; Prednisone belongs to Endocrine.

Evidence level: Low

Why it happens

Mechanism is not firmly established; possible effects on vascular integrity and gastric mucosa add to heparin-related bleeding risk.

What to do

Monitor for signs of bleeding, particularly GI bleeding, and consider gastroprotection in high-risk patients. Use anti-Xa monitoring only in special populations such as renal impairment.
