Summary
Combining a P2Y12 antagonist with a low molecular weight heparin increases the risk of bleeding. This pairing is frequently used deliberately in acute coronary syndromes, so the added risk is accepted but must be managed.
What this interaction means in plain words
Enoxaparin and Ticagrelor can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Enoxaparin belongs to Cardiovascular; Ticagrelor belongs to Cardiovascular.
Why it happens
Additive haemostatic impairment from platelet P2Y12 receptor blockade by ticagrelor and inhibition of coagulation factors Xa and IIa by enoxaparin.
What to do
Use only when there is a clear indication and monitor for bleeding, haemoglobin and platelet count. Limit the duration of parenteral anticoagulation to what is needed and adjust enoxaparin for renal impairment.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
