Summary

Levoketoconazole is a strong CYP3A inhibitor and is expected to markedly increase ticagrelor exposure, raising bleeding risk. Strong CYP3A inhibitors such as ketoconazole are not recommended with ticagrelor.

What this interaction means in plain words

Levoketoconazole and Ticagrelor are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Levoketoconazole belongs to Endocrine; Ticagrelor belongs to Cardiovascular.

Evidence level: High

Why it happens

Strong CYP3A4 inhibition reduces ticagrelor clearance.

What to do

Avoid the combination and choose an alternative antiplatelet. If antifungal or cortisol-lowering therapy is needed, consider an option without strong CYP3A inhibition.
