Summary
Ephedrine with an MAOI can cause severe hypertension and hypertensive crisis. Effects can be markedly greater than expected.
What this interaction means in plain words
Ephedrine and Isocarboxazid are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Ephedrine belongs to Cardiovascular; Isocarboxazid belongs to Central Nervous System.
Why it happens
Ephedrine releases norepinephrine from nerve terminals that are loaded because of MAO inhibition, and metabolism is reduced, producing exaggerated pressor effects.
What to do
Avoid ephedrine, including in cold remedies, during and for 14 days after MAOI therapy. If a vasopressor is needed perioperatively, use a direct-acting agent in carefully titrated small doses with close monitoring.
What to ask your pharmacist
- Is there an alternative to one of these two medicines for me?
- If both are essential, who is monitoring me and how often?
- Which symptoms mean I should stop and seek help straight away?
