Summary
Iloperidone prolongs the QT interval, and epinephrine can provoke tachycardia, hypokalemia and arrhythmias. Together they may raise the risk of torsades de pointes, and iloperidone's alpha-blockade may also cause a paradoxical hypotensive response to epinephrine.
What this interaction means in plain words
Epinephrine and Iloperidone can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Epinephrine belongs to Endocrine; Iloperidone belongs to Central Nervous System.
Why it happens
Additive QT effects with beta-mediated hypokalemia, plus alpha-1 antagonism by iloperidone opposing epinephrine's vasoconstriction.
What to do
Monitor ECG, potassium and magnesium, and blood pressure when both are needed. Correct electrolyte abnormalities and use vasopressors other than epinephrine for hypotension in patients on iloperidone where possible.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
