Summary

Perphenazine has alpha-adrenergic blocking activity, so epinephrine can produce unopposed beta-mediated vasodilation and hypotension. Both agents may also affect cardiac rhythm and QT. This is mostly relevant with systemic doses, for example during anaphylaxis treatment.

What this interaction means in plain words

Epinephrine and Perphenazine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Epinephrine belongs to Endocrine; Perphenazine belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Alpha-adrenergic blockade by perphenazine reverses the pressor effect of epinephrine, with possible additive arrhythmogenic effects.

What to do

Monitor blood pressure and ECG when systemic epinephrine is given to patients taking perphenazine. Use epinephrine as needed in emergencies and use a pure vasopressor such as norepinephrine for refractory hypotension.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
