Summary
Fenoprofen can blunt the blood pressure lowering and natriuretic effects of eplerenone. The combination also increases the risk of hyperkalemia and renal impairment, especially in older, dehydrated or renally impaired patients.
What this interaction means in plain words
Eplerenone and Fenoprofen can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Eplerenone belongs to Cardiovascular; Fenoprofen belongs to Musculoskeletal.
Why it happens
NSAID inhibition of prostaglandin synthesis causes sodium and fluid retention and reduced renal function, antagonizing the diuretic effect and promoting potassium retention.
What to do
Monitor blood pressure, serum potassium and creatinine when starting or changing the NSAID. Use the lowest NSAID dose for the shortest time or choose an alternative analgesic.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
