Summary

Both drugs reduce potassium excretion, so together they can cause clinically significant hyperkalemia. The risk is greater in renal impairment, diabetes, older age or with potassium supplements.

What this interaction means in plain words

Eplerenone and Quinapril can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Evidence level: High

Why it happens

ACE inhibition lowers aldosterone production while eplerenone blocks the aldosterone receptor, giving additive potassium retention.

What to do

Check serum potassium and creatinine at baseline, within the first week, and periodically afterwards. Stop potassium supplements and reduce or stop eplerenone if potassium rises above the accepted limit.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
