Summary
Eprosartan is another angiotensin receptor blocker, so combining it with losartan gives duplicate blockade of the angiotensin II type 1 receptor. This increases the risk of hypotension, hyperkalemia and renal dysfunction without proven benefit.
What this interaction means in plain words
Eprosartan and Hydrochlorothiazide-losartan can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Eprosartan belongs to Cardiovascular; Hydrochlorothiazide-losartan belongs to Cardiovascular.
Why it happens
Duplicate AT1 receptor blockade producing additive hypotension, reduced aldosterone effect and reduced glomerular filtration.
What to do
Avoid combining two angiotensin receptor blockers; use a single agent at an optimized dose. If the combination is unavoidable, monitor blood pressure, potassium and creatinine.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
