Summary
Sulindac can blunt the blood pressure lowering effect of eprosartan and hydrochlorothiazide. With an ARB and a diuretic, NSAIDs increase the risk of acute kidney injury and hyperkalemia, especially in elderly or volume-depleted patients.
What this interaction means in plain words
Eprosartan-hydrochlorothiazide and Sulindac can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Eprosartan-hydrochlorothiazide belongs to Cardiovascular; Sulindac belongs to Musculoskeletal.
Why it happens
NSAID inhibition of renal prostaglandin synthesis causes sodium retention and reduced glomerular perfusion, counteracting the ARB and diuretic.
What to do
Monitor blood pressure, creatinine and potassium after starting sulindac, and keep the patient hydrated. Use the lowest effective dose for the shortest time or choose a non-NSAID analgesic.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
