Summary
NSAIDs such as tolfenamic acid can blunt the antihypertensive effect of angiotensin receptor blockers and thiazides. In combination with a diuretic and an ARB, they raise the risk of acute kidney injury, especially with volume depletion, and may contribute to hyperkalemia.
What this interaction means in plain words
Eprosartan-hydrochlorothiazide and Tolfenamic acid can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Eprosartan-hydrochlorothiazide belongs to Cardiovascular; Tolfenamic acid belongs to Musculoskeletal.
Why it happens
NSAID inhibition of renal prostaglandin synthesis reduces renal perfusion and sodium excretion and impairs potassium handling, which counteracts ARB and diuretic effects.
What to do
Monitor blood pressure, serum creatinine and potassium when starting or stopping the NSAID, and keep the patient well hydrated. Use the lowest effective NSAID dose for the shortest time, or choose paracetamol where appropriate.
What to ask your pharmacist

- Does the dose of either medicine need to change?
