Summary

Both drugs prolong the QT interval in a dose-related way, so together they may increase the risk of torsades de pointes. Risk is greater with higher escitalopram doses, older age, hypokalemia or cardiac disease.

What this interaction means in plain words

Escitalopram and Moxifloxacin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Escitalopram belongs to Central Nervous System; Moxifloxacin belongs to Anti Bacterial.

Evidence level: Moderate

Why it happens

Additive blockade of cardiac hERG potassium channels, prolonging ventricular repolarization.

What to do

Prefer an antibiotic without QT effect. If combined, obtain a baseline and follow-up ECG, correct electrolytes and keep escitalopram at the lowest effective dose.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
