Summary
Both drugs lower blood pressure, and the combination can cause additive hypotension. Beta-blockade may also prevent the reflex tachycardia that normally accompanies dihydropyridine vasodilation, though isradipine has little direct effect on AV conduction.
What this interaction means in plain words
Esmolol and Isradipine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive hypotensive effects from vasodilation by calcium channel blockade and reduced cardiac output from beta-blockade.
What to do
Monitor blood pressure and heart rate and titrate each drug to response. Reduce doses if symptomatic hypotension occurs.
