Summary
Ethacrynic acid is a potent loop diuretic that, with hydrochlorothiazide, produces strong sequential nephron blockade, with substantial risk of hypokalemia, hyponatremia, hypomagnesemia, and volume depletion. Olmesartan may partly offset potassium loss but raises the risk of hypotension and renal impairment when volume is depleted.
What this interaction means in plain words
Ethacrynic acid and Hydrochlorothiazide-olmesartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Ethacrynic acid belongs to Cardiovascular; Hydrochlorothiazide-olmesartan belongs to Cardiovascular.
Why it happens
Additive diuretic and natriuretic effects at different nephron sites with additive potassium loss, while ARB-mediated reduction in aldosterone partly opposes it.
What to do
Monitor electrolytes, renal function, volume status and blood pressure, especially at initiation or dose changes. Adjust diuretic doses or supplement potassium and magnesium as needed.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
