Summary
Loop diuretics combined with ARBs can cause marked hypotension, volume depletion and acute kidney injury, particularly at the start of therapy or in patients who are already dehydrated. Potassium effects oppose each other, as the diuretic wastes potassium while olmesartan retains it.
What this interaction means in plain words
Ethacrynic acid and Olmesartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Ethacrynic acid belongs to Cardiovascular; Olmesartan belongs to Cardiovascular.
Why it happens
Additive reduction in blood pressure and renal perfusion from diuresis and angiotensin II blockade, with opposing effects on potassium handling.
What to do
Monitor blood pressure, creatinine and electrolytes after starting or adjusting either drug. Consider reducing the diuretic dose when starting olmesartan in volume-depleted patients.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
