Summary
Siltuximab blocks IL-6, which can reverse the cytokine-mediated suppression of CYP450 enzymes. Metabolism of CYP substrates such as ethinyl estradiol and levonorgestrel may therefore increase, possibly lowering their concentrations and efficacy.
What this interaction means in plain words
Ethinyl estradiol-levonorgestrel and Siltuximab can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Ethinyl estradiol-levonorgestrel belongs to Endocrine; Siltuximab belongs to Miscellaneous.
Why it happens
IL-6 inhibition restores CYP3A4 and other CYP450 expression, increasing clearance of substrate drugs.
What to do
Advise additional non-hormonal contraception during and for several weeks after siltuximab. Monitor for breakthrough bleeding or loss of contraceptive effect.
What to ask your pharmacist
