Summary

Interleukin-6 suppresses hepatic CYP450 expression during inflammation. Blocking IL-6 with siltuximab may restore enzyme activity and lower cyclosporine concentrations, which could reduce immunosuppression.

What this interaction means in plain words

Cyclosporin and Siltuximab can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Cyclosporin belongs to Miscellaneous; Siltuximab belongs to Miscellaneous.

Evidence level: Moderate

Why it happens

Siltuximab neutralizes IL-6, reversing cytokine-mediated downregulation of CYP3A4 and increasing cyclosporine metabolism.

What to do

Monitor cyclosporine trough concentrations when siltuximab is started or stopped. Adjust the cyclosporine dose according to levels and clinical response.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
