Summary

Fluoroquinolones have been reported in isolated cases to alter phenytoin levels, and ethotoin is a related hydantoin, but no solid data exist for moxifloxacin. Both drugs may also lower seizure threshold or cause CNS effects, so an effect on seizure control cannot be excluded.

What this interaction means in plain words

Ethotoin and Moxifloxacin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Mechanism is not established; extrapolated from case reports of quinolone effects on phenytoin and quinolone CNS excitation.

What to do

Monitor seizure control and consider checking anticonvulsant response if moxifloxacin is started in patients on ethotoin. Choose another antibiotic if seizures worsen.

What to ask your pharmacist
