Summary
Felodipine undergoes extensive CYP3A4 first-pass metabolism and is very sensitive to CYP3A4 inhibitors. Miconazole may raise felodipine levels, leading to hypotension, flushing, headache and peripheral edema. Vaginal use reduces but does not remove this risk.
What this interaction means in plain words
Felodipine and Miconazole can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Felodipine belongs to Cardiovascular; Miconazole belongs to Anti Fungals.
Why it happens
Miconazole inhibits intestinal and hepatic CYP3A4, decreasing felodipine clearance.
What to do
Monitor blood pressure and for edema or dizziness during miconazole therapy. Reduce the felodipine dose if hypotension or marked vasodilatory effects occur.
