Summary

NSAIDs such as fenbufen can blunt the antihypertensive effect of losartan and impair renal function, especially in elderly, dehydrated or renally impaired patients. Hyperkalemia risk is also increased.

What this interaction means in plain words

Fenbufen and Losartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: High

Why it happens

NSAID inhibition of renal prostaglandin synthesis reduces renal perfusion, sodium excretion and potassium excretion, counteracting ARB effects.

What to do

Ensure adequate hydration and monitor blood pressure, serum creatinine and potassium, particularly at initiation. Use the lowest effective NSAID dose for the shortest time or choose an alternative analgesic.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
