Summary

Sparsentan is a dual endothelin and angiotensin II receptor antagonist, so adding an ARB duplicates angiotensin receptor blockade. This increases the risk of hypotension, syncope, hyperkalemia and renal function decline.

What this interaction means in plain words

Losartan and Sparsentan are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Losartan belongs to Cardiovascular; Sparsentan belongs to Genitourinary.

Evidence level: High

Why it happens

Duplicate angiotensin II type 1 receptor blockade producing additive pharmacodynamic effects.

What to do

Do not use together; stop the ARB before starting sparsentan. Monitor blood pressure, potassium and renal function after initiation.

What to ask your pharmacist
