Summary
NSAIDs can reduce renal clearance of aminoglycosides and add to nephrotoxicity, leading to higher gentamicin concentrations and increased risk of kidney injury and ototoxicity. This has been documented particularly in neonates and in renal impairment.
What this interaction means in plain words
Fenoprofen and Gentamicin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Fenoprofen belongs to Musculoskeletal; Gentamicin belongs to Anti Bacterial.
Why it happens
NSAID inhibition of renal prostaglandins reduces renal blood flow and glomerular filtration, decreasing aminoglycoside elimination and adding nephrotoxic injury.
What to do
Avoid the NSAID during aminoglycoside therapy where possible; otherwise monitor gentamicin levels and serum creatinine closely and adjust dosing.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
