Summary

Iloperidone is mainly a CYP3A4 and CYP2D6 substrate and is not a recognized meaningful CYP3A4 inhibitor, so a kinetic effect on fentanyl is unlikely. The combination does add CNS depressant and hypotensive effects, with sedation and orthostasis possible.

What this interaction means in plain words

Fentanyl and Iloperidone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Fentanyl belongs to Musculoskeletal; Iloperidone belongs to Central Nervous System.

Evidence level: Low

Why it happens

Additive CNS depression and hypotension from the antipsychotic and opioid; no established CYP3A4 inhibition.

What to do

Monitor for sedation, orthostatic hypotension and respiratory depression, starting with lower doses of each agent. Counsel patients on fall risk.

What to ask your pharmacist
