Summary

Fingolimod slows heart rate and can prolong the QT interval, particularly at treatment initiation. Salbutamol is a beta-agonist that can cause tachycardia, hypokalemia and arrhythmia, especially at high doses. Together they may add to the risk of rhythm disturbances.

What this interaction means in plain words

Fingolimod and Ipratropium-salbutamol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Fingolimod belongs to Central Nervous System; Ipratropium-salbutamol belongs to Respiratory.

Evidence level: Low

Why it happens

Additive effects on cardiac conduction and repolarization, with salbutamol-induced hypokalemia increasing arrhythmia susceptibility.

What to do

Monitor ECG and heart rate, particularly during fingolimod initiation, and check potassium if salbutamol is used frequently or at high doses. Avoid excessive salbutamol use.

What to ask your pharmacist
