What this interaction means in plain words

Fingolimod and Natalizumab can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Fingolimod belongs to Central Nervous System; Natalizumab belongs to Gastrointestinal Tract.

Evidence level: Moderate

Why it happens

Additive immunosuppression: natalizumab blocks alpha-4 integrin-mediated leukocyte trafficking while fingolimod sequesters lymphocytes in lymph nodes.

What to do

Avoid concurrent use. When switching, monitor for infection and PML symptoms, and allow for the lingering immune effects of the first drug.
